First, you will need to obtain a copy of the request for medical payment form. You can download and print a copy of the form online on the Tricare website. Once you complete the form, you must sign it.

Next, you must attach a readable copy of the provider’s bill to your signed payment form. Information that must be included includes the provider’s name and address, and the date, place, description, diagnosis, and amount charged for each service.
